Rehearsals are held on Tuesday and Thursday evenings and Saturday mornings/afternoons. Please list all possible conflicts with this rehearsal schedule. Be sure to include all possible obligations (school, work, special events, etc.). If no conflicts exist, please enter "none."

Fantasy Playhouse Children's Theater productions are staffed with volunteers. We are always welcoming new members to the Fantasy Family, and experience is not required! Please note any tasks that you or a family are willing to assist with.

Proper behavior is expected and therefore we reserve the right to remove a performer from a production due to inappropriate or disruptive behavior. Submission of this form also signifies consent to grant our photographer/videographer the right to record, edit, use reproduce, publish, and distribute by way of photograph, video, television, and all other media (electronic or otherwise) the visual and/or audio likeness of participants in the above-referenced project.

I, the above-registered participant (the “Participant”), desire to participate in classes, camps, or volunteer activities (the “Activity”) at a the 3312 Long Avenue building, the 2113 Holmes Avenue scene shop facility or any offsite location leased by Fantasy Playhouse Children’s Theater & Academy. (the “Company'). As lawful consideration for being permitted by the Company to participate in the Activity, I agree to all the terms and conditions set forth in this Release (this “Release").
I AM AWARE AND UNDERSTAND THAT: (A) REGARDLESS OF THE PRECAUTIONS TAKEN BY THE COMPANY, MYSELF, AND OTHER PARTICIPANTS, I MAY BE EXPOSED TO AND POTENTIALLY CONTRACT COVID-19; AND (B) I AM VOLUNTARILY PARTICIPATING IN THE ACTIVITY WITH KNOWLEDGE OF THE POTENTIAL EXPOSURE RISK INVOLVED AND HEREBY AGREE TO ACCEPT AND ASSUME ANY AND ALL RISKS ASSOCIATED WITH SUCH EXPOSURE, WHETHER CAUSED BY THE NEGLIGENCE OF THE COMPANY, OTHER PARTICIPANTS OR OTHERWISE.

In the event that any provision of this Release is adjudicated to be unenforceable, I agree that such a determination shall, in no way, affect the validity and enforceability of the remaining provisions of this Release, all of which shall remain enforceable to the fullest extent permitted by law.
